When you’re out drinking and having a good time, it’s tempting to get in the car and drive yourself home. However, that decision can have serious consequences—a DUI, lost job, an accident, injuring yourself or others, or even worse. Drinking and driving laws in Arizona are especially stringent:

A DUI in Arizona can cost in excess of $10,000 (including attorney fees and penalties).
Offenders must have an ignition interlock device installed on their car for at least six months.
DUIs impact every aspect of your life, not just transportation.

Here is how it works

Step 1

Once you receive your Key4Life fob, attach it to your keychain, and always carry it with you.

Step 2

Next time you’re out drinking and need a ride home, call 888-311-8111, and give the agent the serial number from your key fob sticker to dispatch a taxi.

Step 3

When the taxi arrives, give the driver the serial number sticker from the front of your key fob. A tip is optional and always appreciated.

Please note:

The taxi must pick you up at a public place that sells or serves alcohol and take you directly to a residence.
You may have one other person share your ride home (two people per taxi).
Each Key4Life fob has a unique serial number sticker that is good for ONE ride only.

Program terms, conditions, and limitations

The Don’t Drink and Drive Key4Life program (the “Program”) is designed to give you another option when you or a companion is in need of a Safe Ride Home. The Key4Life Fob is placed on your keychain so that when you are out enjoying yourself at a public place, restaurant or bar that sells alcohol and either you or a companion has been drinking, is impaired by drugs, intoxicated, or otherwise unable to drive, the Key4Life sticker with serial number on the Key4Life fob can be exchanged for a ride from VIP Taxi, LLC (hereafter, “Key4Life”) for up to two people to a single home or residence: (1) from a location within the Valley of the Sun to another location in the Valley of the Sun (Phoenix Metropolitan Area), or; (2 ) from a location within the Tucson Metropolitan Area to another location in the Tucson Metropolitan Area, regardless of the distance of the ride.

By receiving or requesting a Key 4 Life Fob/Key4Life Sticker combination you acknowledge and agree to the following Program Terms, Conditions and Limitations:

The Program is not to be used for ordinary taxi cab rides but to provide a safe way home from a public place, restaurant, night club, lounge, or bar that sells alcohol if you or a companion has been drinking and/or the person who would otherwise drive is impaired, intoxicated, or otherwise unable to drive. The Key4Life Fob/Sticker with serial number combination is good for only one ride for up to two people traveling from a single pickup point that is a public place that sells alcohol directly to a single drop-off point that is a residence. The price for the Fob/Sticker combination is comprised of several elements including but not limited to Administration Fees, Priority Service and Transportation.

The current breakdown of fees are as follows: Cost apportioned towards transportation $19.95 Cost apportioned towards administration $6.00 Cost apportioned towards service prioritization $4.00 The total cost of a fob/sticker combination $29.95 plus any applicable taxes.

Key4Life reserves the right to cancel program to any or all individual(s) without any further obligation to you. If the Key4Life Sticker has expired, Key4Life, at its option, can exchange it for a new one for a service fee plus the difference between cost of original Key4Life Fob/Key4Life Sticker combination and the cost of the Key4Life Fob/Key4Life Sticker combination at the time of the exchange. The Key4Life Fob holder is responsible if the Fob and/or Sticker are lost or stolen and they will not be replaced. Key4Life reserves the right not to accept, or re-issue any key fob or to otherwise limit its use if Key4Life reasonably believes that the use is outside the scope of its intended use, unauthorized, fraudulent, or otherwise unlawful.

The Program is designed to provide only basic transportation services; no medical or healthcare services or additional assistance are provided by the Program or the Independent Contractor Driver. The time you wait for the cab depends on availability at that time of your call, Key4Life will make reasonable efforts to send the cab to your pick-up location on a priority basis. Key4Life is not responsible for any injuries that you, or any other passenger may sustain entering the cab or anytime before the taxicab ride starts or anytime after the conclusion of the ride including while the passenger(s) exit the taxicab and any and all time after. The Program and Key4Life may only be responsible for injuries to you or your passenger from its negligence while you are in the taxicab. You and your passenger assume all risk for activities, acts or omissions that occur outside of the taxicab and for your or your companions’ carelessness, gross negligence or willful wrongdoing while inside the cab. Passengers that get sick or otherwise damage or dirty the taxicab before, during or after the ride will be responsible at their expense, to pay the Independent Contractor Driver at the conclusion of the ride full restitution to clean, fix or repair the taxicab including any losses of income incurred during the period to fix, clean or otherwise repair.

Key4Life may amend these Terms, Conditions, and Limitations at any time. As permitted by applicable law, any amendment will become effective at the time we post the amended agreement on the Key4Life web site (www.yourkey4life.com) or as otherwise stated in our notice to you. The amendment will apply to your future and existing Key4Life Fob/Sticker combinations. The Program and these Terms, Conditions and Limitations and your relationship to the Program are governed and controlled by the laws of the State of Arizona without reference to conflict-of-laws principles.

KEY4LIFE MAKES NO REPRESENTATIONS OR WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED. Key4Life may assign all or any part of rights under the Program or this agreement without notice to you. It will then be released from all liability. These terms, conditions, and limitations together with the other provisions of this brochure constitute the complete and exclusive statement of agreement between you and Key4Life.
